Police Life Insurance - Sworn Benefit Nomination Form

This form nominates the Partner to whom the Life Benefit will be paid to in the event of your death and (for Police Welfare Fund members) provides a Life Benefit to you, in the event of their death. If your personal circumstances change, a new form should be completed immediately.
