Receive additional Life Insurance with reduced underwriting!
We have arranged a reduced underwriting facility to enable you to purchase additional $50,000 or $100,000 of Life Insurance under the Life Extra Plan. This may help to ensure that your family has enough money in the event of your untimely death – things like credit card bills, mortgage payments, funeral and memorial costs and time off work.
This offer is for you to purchase Life Insurance, additional to what is provided to you under the Sworn Plan. As a member of the New Zealand Police Association, you are provided with up to $300,000 of Life Insurance.
The amount you’re covered for could be paid out early if you’re diagnosed with a terminal illness and not expected to live more than 12 months.

Simple Application
Acceptance of additional cover for you is subject to the satisfactory completion of the simple application and health declaration below. You may be required to provide more information to an AIA underwriter if you answer yes to any of the below questions.
Provider
The cover is provided by AIA New Zealand and has been negotiated by the New Zealand Police Association. The Association is responsible for the day-to-day administration, and negotiation of the best terms for members.
When will cover start?
Subject to the satisfactory completion of your application form, cover will commence when you receive your updated member certificate and make your first premium payment. If your application is approved, the standard premium cost will be deducted from your pay as per your other deductions.

Disclosure information to AIA New Zealand
Definition: “AIA” shall mean AIA New Zealand Limited, and/or any related companies and/or any related companies and/or agents (including company officers acting in the scope of their authority) and AIA’s Insurance consultants or reinsurers.
You are not insured:
- until this application has been accepted by AIA; and
- a Certificate of Insurance is issued to you.
AIA may decline this application, or may accept this application subject to certain conditions and exclusions.
Your duty of disclosure: When you apply for insurance with AIA, you have a legal duty of disclosure to AIA.
This means that:
- All the statements you make to AIA (both written and oral) including the answers in this application, must be true and correct; and
- You must disclose everything that you know, or could reasonably be expected to know, that is relevant to AIA’s decision whether:
- to accept your application for insurance; and
- if AIA accepts your application, then on what terms AIA will accept it and how much it will cost.
- This duty of disclosure continues from the time you complete this application until either:
- the commencement date of this policy or the date AIA accepts your application for insurance, whichever is the later; or
- AIA declines your application for insurance.
- You also have the same duty of disclosure to AIA at the time you extend, vary or reinstate your insurance.
Important
If you do not comply with your duty of disclosure, and AIA New Zealand would not have accepted your application for insurance on the same terms or at the same premium if you had made full disclosure, then legally AIA New Zealand may:
• decline any claim that you make; and/or
• retain all premiums paid and recover any benefits paid; and/or
• alter the terms of any benefits under the policy; and/or
• remove any benefits under the policy; and/or
• avoid your insurance from inception

Declaration to AIA
It is important for you to read and understand this declaration before signing the application, as there are terms and conditions that you may not be aware of and that will form part of your insurance if AIA accepts your application.
- I declare that the statements made in this application are true and complete and that I have disclosed all information material to this insurance for myself and on behalf of family members.
- I agree that this application and any other written statements made in connection with the proposed insurance shall form the basis of the contract between myself and AIA.
- I understand that AIA reserves the right to recover any medical costs incurred in assessing this application should I decide to cancel this application.
- I further declare that if the answers to the questions in this application are not in my writing, that they have been correctly written down at my dictation and read and approved by me.
- I acknowledge that if I undergo any alteration in my mental or physical health or have a change of occupation between the date of this application and the commencement date of this policy, or the date AIA accepts this application for insurance, whichever is later, I agree to notify AIA immediately.
- I acknowledge that personal information collected or held by AIA (whether contained in this application or otherwise obtained) is provided and may be held, used, and disclosed by AIA to:
- process this application; and
- any other application for insurance I make to AIA; and
- for the purpose of assessing any claim I may make should this or any other application be accepted by AIA
- for the purposes of any legal proceedings before a Court, or review or arbitration before a statutory or independent body.
- I acknowledge that if I fail to provide any information requested in this application, AIA may be unable to process the application for insurance.
- I understand that access to my personal information is available to me under the Privacy Act 2020 by writing to AIA.
- I authorise AIA to obtain personal information held about me relevant to my application, my insurance, or any claim that I may make. This declaration shall constitute sufficient authority to the party that AIA requests the information from and extends to personal information held about me by any government department, incorporated body or person, including (but not limited to) information held by:
- Accident Compensation Corporation
- accountants and other financial advisers
- banks and insurers
- counsellors, psychologists and therapists
- dentists
- employers
- government departments and bodies
- medical laboratories
- Police Health Plan Ltd
- Police Welfare Fund Insurances Ltd
- private and public hospitals
- registered medical practitioners and specialists
- I agree that a photocopy of this authority shall be treated as an original.
Financial Strength Rating: AIA New Zealand Limited has been given an AA (Very Strong) insurer financial strength rating by Fitch Ratings, an approved ratings agency. A rating of AA means AIA New Zealand Limited has a very strong capacity to meet policyholder and contractual obligations.
Ratings scale: AAA-Exceptionally strong, AA-Very strong, A-Strong, BBB-Good, BB-Moderately Weak, B-Weak, CCC-Very weak, CC-Extremely weak, C-Distressed.
Note: "+" or "-" may be appended to a rating to indicate the relative position of a credit within the rating category. Such suffixes are not added to ratings in the AAA category or to ratings below the CCC category.
